When comparing TEG to ROTEM… Thromboelastography is a method of testing the efficiency of blood coagulation. It is a test mainly used in surgery and anesthesiology, although increasingly used in resuscitations in Emergency Departments, intensive care units, and labor and delivery suites. More common tests of blood coagulation include prothrombin time and partial thromboplastin time which measure coagulation factor function, but TEG …

•Adventage : Real-time analyse of clot formation and dissolusion coagulopathy by TEG or ROTEM allows a timely preparation (thawing) of blood products or a prompt intervention using plasma-derived or recombinant factor concentrate.4-6,36,37 COAGULATION TESTING ON ROTEM The basic principles and technical aspects of TEG and ROTEM have been reviewed elsewhere.14,38-40 In this article, Thromboelastography is a mechanism of assessing coagulation based upon the viscoelastic properties of whole blood, and when compared to other coagulation assessments, this test allows for an assessment of near real-time, in-vivo clotting capacity.

When comparing suggested blood products using our TEG decision tree and an Intem based decision tree, both devices had the best agreements on transfusion of no products as guided by conventional tests, but lower agreement with abnormal conventional tests that would have resulted in blood component transfusion based on low fibrinogen levels (TEG and ROTEM- 0%), low platelets (TEG 44.4%, ROTEM 54.5%), or elevated PT or PTT (TEG 0%, ROTEM 22.2%). Repeat ROTEM analysis 10 mins after any intervention to assess response.

Staff ROTEM is a modern modification of the TEG technology originally described by Hertert in 1948 [1]. These technologies provide a visual assessment of clot formation and subsequent lysis under low shear conditions (0.1/sec) similar to Physically speaking, both ROTEM and TEG involve the oscillating motion of a cylindrical shaft through a small sample of blood. As the blood forms into a clot, it exerts increasing resistance on the shaft. By measuring this resistance, we are able to track the strength of the forming clot over time. Rotational thromboelastometry (ROTEM) evolved from TEG technology and both devices generate output by transducing changes in the viscoelastic strength of a small sample of clotting blood (300 µl) to which a constant rotational force is applied. THROMBOELASTOGRAPHY (TEG) IN TRAUMA SUMMARY Thromboelastography (TEG) is a test of whole blood coagulation that was developed in the 1950’s, but was largely passed over in favor of conventional coagulation tests (PT, PTT, platelet count). It has been Thromboelastography is a mechanism of assessing coagulation based upon the viscoelastic properties of whole blood, and when compared to other coagulation assessments, this test allows for an assessment of near real-time, in-vivo clotting capacity.

The point-of-care assay, which uses graphic interpretation of thromboelastography (the TEG), offers the opportunity for an expedited assessment of Of the 4 types of TEG assays available, the most common is the rapid TEG, and it is the assay referred to in this review. The use of an activator in rapid TEG standard-izes the TEG test and speeds up the rate at which clotting takes place, thus making results available more quickly.9 The first measurement of note is the reaction time (R time).
